Additional forms/paperwork are required for High School and Drivers Education. Check these programs below for those specific additional documents and be sure to bring them with you on check-in day.
In addition to the three document above, students signed up for the High School summer program must bring the following two additional forms for a total of five forms required for check-in. (If your student is in high school but is signed up for Drivers Education summer program, you do not need these forms. Check the section of Drivers Education for the additional requirements for that program.)

*With staff assistance, high school students will be using Austin’s Capital Metro city bus transportation while at Summer Programs…learning how to use public transportation, use Google maps to get from point A to point B, show traveling etiquette, and navigating to and from businesses in and around Austin (including, Travis County Services for the Deaf and Hard of Hearing) and the like.
In addition to the three documents required of all summer programs students, both of the above Drivers Education Programs have additional forms/items that are required by the Department of Public Safety for issuing drivers learning permits or drivers licenses.
You must have these items with you on check-in day. All documents are the same for both programs EXCEPT for a physician's statement that in only required for the students in Driver and Safety Education program.
The following three required forms will be in your acceptance packet and available at check-in. It will save you time if you fill them out (in black ink) before you come. However, YOU MUST NOT SIGN THEM! These documents must be signed in the presence of a Notary Public. TSD will provide this free service at check in.
The Department of Public Safety requires the following documentation. These must be left with us at TSD as DPS will not accept copies. For identification protection, TSD will lock up these documents for safe-keeping. The documents will be returned to you at the end of Summer Programs.
A Physician’s statement, signed by your doctor, is required for students in the Driver and Safety Education Program but NOT for Adult Driver Education students.
Finally, the student must arrive at check-in with
