Three forms are required for ALL summer camps & programs.

Campers and registrants for STEM Camp, I Can! Learn STEM program, and I Can! Code program are also required to submit a Program Interest Form and a Teacher Recommendation Form in addition to the three forms required of all summer camps & programs. The number of available slots for these camp and programs is limited and selection is competitive. Those who are registered in the I Can! Learn STEM and I Can! Code programs will not be able to also register for the I Can! Drive program sessions.
I Can! Drive (Teen Driver Education - for ages 15-17 only) and I Can! Drive (Adult Driver Education - for ages 18-22 only) programs have additional forms that are required by the Department of Public Safety for issuing drivers learning permits or drivers licenses.
You must have these forms with you on check-in day in addition to the three forms required of all summer camps & programs. All documents are the same for both I Can! Drive programs EXCEPT for an additional form, the Physician's Statement, that needs to be signed by a doctor and is required for driving students ages 15-17.
There are three required forms that will be in the acceptance packet and available at check-in. It will save you time if you fill them out (in black ink) before you come. DO NOT SIGN the following three forms! They must be signed in the presence of a Notary Public, which will be available free of charge during check-in at Texas School for the Deaf.
When you sign this form in the presence of the notary at TSD, the signature MUST MATCH the name that is on the proofs of residency (see below). The legal guardian signature and address must be the same on all these four forms: Proofs of Residency, Texas Residency Affidavit, DL-14A, and DL-40. Also, that same person must be present with the student at time of check-in. Please see the checklist on the envelope, which will be included with your acceptance packet.
Two documents proving residency are required. Please see the samples of what will be accepted at DPS on the back of the Texas Residency Affidavit form. If using an electric, water, natural gas, satellite television, cable television, or non-cellular telephone statement must be dated within 90 days of the date of application (or the first day of Summer Camps & Programs).
The Department of Public Safety requires the following original documentation. These must be left with us at TSD as DPS will not accept copies. For identification protection, we will lock up these documents for safe-keeping until the end of Summer Camps & Programs when they will be returned back to the legal guardian.
Texas School for the Deaf Student Health Services’ Physician’s Statement form, signed by your doctor, is required for driving students in the I Can! Drive (Teen Driver Education - for ages 15-17 only) but it is NOT required for I Can! Drive (Adult Driver Education - for ages 18-22 only).
Finally, the participant must arrive at check-in also with:
